At Angels Care Home Health, WE SERVE PATIENTS. For us, this is more than the founding philosophy that our ownership built this great company on. It is the commitment we make to every patient, every caregiver and every referral source in each of the communities we support. Home health offers the option of a higher quality of life for patient populations that are often rendered powerless due to disease states that they don’t understand nor are they capable of controlling alone. We view each patient referral as an opportunity to make a difference in a person’s quality of life. This opportunity comes with the great responsibility of providing our patients with an optimal environment for complete healing in the place where it makes the most sense…their own home. What We Do

We provide skilled nursing care, restorative therapy and medical social services to patients in their homes or wherever they may reside, including assisted living facilities and retirement communities.

Under the direction of the physician, we administer medical services to the patient while strictly adhering to the physician’s plan of care. Our services create a continuum of care throughout the disease process.

We emphasize patient and family education, which empowers the patient to take an active role in the management of their condition. This is supported by our Community Classroom and Caregiver Support Education classes conducted each month in the communities we serve.

Our Difference

We admit patients 24 hours a day, seven days a week. When our patients are first admitted onto service, they receive more visits from nurses and therapists in the first two weeks than is typical of the average home health agency.

This extra support in the initial healing process is crucial in assisting the patient to quickly regain their confidence and independence. Education through our disease management programs also distinguishes us.

Whether our patients are coping with chronic illness, healing from an injury, or relearning lost skills, they have the resources to help them manage their condition and enjoy quality living at home.
